This self-paced training prepares community health workers, home health aides, care coordinators, social workers, and other frontline professionals to identify older adults at risk for falls, conduct evidence-based fall risk screenings and functional assessments, document findings, and connect individuals with appropriate community and healthcare resources.
Syllabus
Course Overview:
This training equips community health workers, home health aides, care coordinators, social service professionals, and other frontline staff with the practical knowledge and skills to:
- Conduct evidence-based fall risk screenings.
- Perform standardized functional mobility assessments.
- Identify home and environmental fall hazards.
- Recognize common medical and behavioral fall risk factors.
- Document assessment findings appropriately.
- Connect older adults with evidence-based programs and community resources using Missouri-specific referral pathways.
Learning Outcomes:
By the end of this training, you will be able to:
- Conduct evidence-based falls risk screenings using validated tools.
- Perform standardized functional mobility assessments safely and accurately.
- Recognize intrinsic, extrinsic, environmental, and behavioral fall risk factors.
- Identify home hazards and recommend practical safety modifications.
- Document assessment findings using best practices.
- Connect older adults with healthcare providers, evidence-based programs, and community resources using Missouri referral pathways.
- Support older adults in reducing their fall risk and maintaining independence.
