Living a Healthy Life with Chronic Conditions programs are six-week, highly interactive group programs to help people with chronic disease(s) and caregivers of people with chronic disease(s) gain self-confidence in their ability to control their symptoms and learn how their health problems affect their lives. Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. These evidence-based programs were developed at Stanford University and are now managed by the Self-Management Resource Center.  

Living a Healthy Life with Diabetes (DSMP) helps people with type 2 diabetes and caregivers of people with type 2 diabetes manage symptoms including hyper- or hypoglycemia, tiredness, pain, and emotional issues. Participants learn skills to better manage diabetes day-to-day.


Cost: $0 per participant thanks to funding from the Missouri Department of Health and Senior Services diabetes program
