Living a Healthy Life with Chronic Conditions programs are six week, highly interactive group programs to help people with chronic disease(s) and caregivers of people with chronic disease(s) gain self-confidence in their ability to control their symptoms and learn how their health problems affect their lives. Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. These evidence-based programs were developed at Stanford University and are now managed by the Self-Management Resource Center.

  • Living a Healthy Life with Chronic Conditions (CDSMP) topics include dealing with feelings of fear and isolation, appropriate exercise, medication use, nutrition, managing depression, how to evaluate new treatments, and communicating with family, friends and health team.
  • Living a Healthy Life with Diabetes (DSMP) helps people with type 2 diabetes and caregivers of people with type 2 diabetes manage symptoms including hyper- or hypoglycemia, tiredness, pain, and emotional issues. Participants learn skills to better manage diabetes day-to-day.
  • Living a Healthy Life with Chronic Pain (CPSMP) is designed to help individuals with chronic pain better manage their symptoms by learning important self-management skills and techniques such as action planning, healthy eating, pacing and rest, communicating with their healthcare team and more. Participants receive the Moving Easy Program as part of this class.
  • Living a Healthy Life at Work (wCDSMP) is designed to help working adults better manage chronic disease symptoms by learning important self-management skills and techniques such as action planning, healthy eating, communicating with their healthcare team and more. This program provides techniques for balancing work and home life.

Cost: $60 per participant

Sessions:
(start date - end date, day(s) of week; six weeks for all unless adding an orientation session; include both days of the week if teaching wCDSMP)

Time:
(start time - end time; 2.5 hours for in-person/virtual CDSMP, DSMP, CPSMP; 1 hour for all forms of toolkits and wCDSMP in-person or virtual)

Location:
