Living a Healthy Life with Chronic Conditions programs are six week, highly interactive group programs to help people with chronic disease(s) and caregivers of people with chronic disease(s) gain self-confidence in their ability to control their symptoms and learn how their health problems affect their lives. Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. These evidence-based programs were developed at Stanford University and are now managed by the Self-Management Resource Center.
Living a Healthy Life at Work (wCDSMP) is designed to help working adults better manage chronic disease symptoms by learning important self-management skills and techniques such as action planning, healthy eating, communicating with their healthcare team and more. This program provides techniques for balancing work and home life.
Cost:
$25 per participant*
*This program is being offered at no cost to MU employees and MU Extension County Council members.
