The Chronic Disease Self-Management Program (CDSMP) is a six week, highly interactive group program to help people gain self-confidence in their ability to control their symptoms and learn how their health problems affect their lives. 

Topics include

  • Dealing with feelings of fear and isolation
  • Appropriate exercise
  • Medication use 
  • Nutrition 
  • Managing depression
  • How to evaluate new treatments
  • Communicating with family, friends and health team

CDSMP is an evidenced based program developed at Stanford University and now managed by the Self-Management Resource Center.
