Please fill out our online Sample Analysis Selection form on the page below or you can access the full-screen sample analysis form here. If you have sets of samples that require different analyses, complete separate submission forms.
If you require an official letterhead quote, contact SHAC via phone or email after filling out your sample submission. If you do not need a formal quote, try our Price Estimate form to anticipate your costs!
The DNR SWCD Cover Crops Cost Share Program forms are available for the 2025-2026 fiscal year as printable PDFs: Initial Samples form (PDF) and Follow-up Samples form (PDF).
Sample Analysis Selection Form
Average sample turnaround time is currently 4 months.
This form is provided for clients to submit sample information prior to sending samples so the SHAC team can be prepared when the samples arrive. We will not begin processing samples without this form being submitted. Sample drop-off is from 7:30 a.m. to 4 p.m. Monday through Friday.
