Workplace Chronic Disease Self-Management


The Workplace Chronic Disease Self-Management Program is an evidence-based six-week program for working adults living with one or more chronic conditions. It is designed to help individuals better manage their symptoms by learning important self-management skills and techniques such as action planning, healthy eating, communicating with their healthcare team and more. This program was originally developed at Stanford University and is now managed by the Self-Management Resource Center. Participants will receive a free Living a Healthy Life resource book and relaxation CD for enrolling.
