Living Healthy with Diabetes - DSMP - Faith Community Health Clinic - Branson

Description

The Diabetes Self-Management Program is a six-week program for people with type 2 diabetes and is designed to help participants manage their diabetes symptoms by learning skills to better manage their diabetes day to day. This is an interactive, evidence-based workshop developed at Stanford University. Partners and caregivers are encouraged to attend.

Topics include:
>Making healthy food choices and planning menus
>How to increase physical activity and exercise safely
>Preventing or delaying diabetes complications
>Communicating effectively with your health team
>Developing action plans and setting goals to improve your health and well-being

The program is FREE but registration is required. Register online or call Stone County MU Extension Office Manager, JoEtta Bowling at (417) 357-6812. 

Participants will receive a FREE Living a Healthy Life with Chronic Conditions book and relaxation CD.
