Living a Healthy Life with Chronic Conditions in the Workplace

Description

This program is an adaptation of the 6-week Chronic Disease Self-Management Program. It contains similar content, but is rearranged into 1-hour segments for the workplace. Small groups of peers meet twice a week for 1 hour to help each other solve problems they encounter in creating and carrying out their self-management program. Participants make weekly action plans, share experiences, and learn about a variety of topics including:

Techniques to deal with problems such as frustration, fatigue, pain and isolation

  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Decision-making
  • How to evaluate new treatments
  • And much more

This is an evidenced based program developed at Stanford University and now managed by the Self Management Resource Center. Participants receive a free Living a Healthy Life with Chronic Conditions book for participating.
