Living a Healthy Life with Chronic Conditions - Gateway Region YMCA
The Chronic Disease Self-Management Program (also known as Living a Healthy Life) is a six week, highly interactive group program to help people gain self-confidence in their ability to control their symptoms and learn how their health problems affect their lives. Topics include dealing with feelings of fear and isolation, appropriate exercise, medication use, nutrition, managing depression, how to evaluate new treatments, and communicating with family, friends and your health care team. This is an evidence-based program developed at Stanford University and now managed by the Self Management Resource Center. Participants receive a free Living a Healthy Life book for enrolling.
