Diabetes Self-Management Program
There are several low-cost evidence-based programs that show promise to help older adults with chronic illnesses to take control of their own health (Centers for Disease Control and Prevention, 2011). The 6-week Diabetes Self-Management Program (DSMP) developed at the Stanford Patient Education Research Center is one of the most well-studied evidence-based programs and has been selected for widespread national dissemination. In addition, the American Diabetes Association identifies this program as meeting the Diabetes Support Initiative criteria for support programming.
This class will take place in-person at the Jim Sears Building at 55682 State Highway 6 in Edina, MO. Virtual participation is possible.
