The performance expectations/self evaluation form combines information for performance expectations and the reporting of program impact and accomplishments and other information for the self evaluation.
The performance expectations portion of the form is used to define the major areas of planned effort to be initiated during the performance period. This form is to be completed by the regional specialist prior to meeting with their regional director. Review position description, county and individual plans of work, past performance appraisals, situational variables, past and planned participation in PIEs/PDEs, and performance evaluation criteria to determine a set of performance expectations. Based upon review and approval of the Regional Director, these expectations will provide the basis for the annual performance evaluation. Major changes in performance expectations may be made by revising this document and obtaining the approval of the regional director.
The self evaluation portion of the form is used to report all of your significant program impacts and accomplishments and other accomplishments during the evaluation period. Explain why they are important and reference reports you have submitted that provide more details and explain impacts and accomplishments related to PIE expectations (identify PIE by name). Insert the self evaluation information after each performance expectations in bold face type. Self evaluation information, not related to a performance expectation, should be placed at the end of the appropriate section (use bold face type).
A) Planned Program Efforts and Intended Client Impact (50%)
(Briefly describe each major planned program effort and the intended impact on target audience):
1) List all major programs/educational activities planned for the year. Include the following information: name of program/activity, number and type of activity, target audience, number of participants, and anticipated impact on clientele. Specifically identify all activities related to conducting PIE expectations and objective(s) for affirmative action.
2) List actions that you plan to implement that will improve program productivity and client impact with respect to the following criteria: impact, quantity, innovation, quality, and audience.
B) Professional Development and Behavior (20%)
(Briefly describe major planned efforts including PDE/PIEs designed to increase your professional core competence and/or bring about positive changes in your professional behavior.) For self evaluation list all PIE/PDE (including individualized) experiences and professional development activities and explain how each one impacted performance.
C) Teamwork (15%)
(Briefly describe program efforts that you expect to work with others to accomplish goals. What role(s) will you play:
D) Leadership (10%)
(Briefly describe plans for contributing to program development, public relations, marketing, resource development, and/or other activities that contribute to the improvement of University Extension):
