Tools
Voluntary demographic information collection
Each person involved in state or local extension programming should report the demographics of the audiences served, as required by USDA. Voluntary Demographic Information forms may be used to collect this information.
Forms should be given to each program participant. It is not appropriate to guess the demographic information for part of a group and give the form to others for completion. Participants are not asked for names or other personal information, so forms should be collected in a manner in which participants don't feel as though they are being singled out or identified personally. Information collected may be used for affirmative action reporting in WebApps.
