Reporting Requirements

Progress Reports
Progress reports must be submitted for all projects/programs receiving Outreach Development Funds. These reports should show the progress made in the project/program outcomes (refer to reporting format).  Project directors are responsible for ensuring progress reports are posted to the ODF WebBoard  according to the following schedule:

Failure to submit progress reports could result in funding being withheld.

Annual Report
Each year the project director must submit an annual report that summarizes the accomplishments for the year.  The annual report format is the same as with the progress report.  This report is due July 30.

Final Report
A final report is required for each project/program at the end of the approved funding period.  Projects/programs will not be approved to continue beyond the specified duration.  A new proposal may be submitted following Outreach Development Fund guidelines.

Project directors should post the final report on the project/program's web page as well as the ODF WebBoard.  This report should be submitted in accordance with the specified format and within two months of the project/program's ending date.  For example:

If the project/program's ending date is June 30, 2001, the final report will be due no later than August 31, 2001.

Project/Program Update

  • Inputs – Report on whether the program inputs that were initially planned have materialized as expected.  Indicate the FTE of staff devoted to the project/program from the field, campus, and partners.  Report on progress towards the timeline, explaining any significant deviations from plans.  Report on how technology was used.
  • Outputs – Report on activities that have been conducted.  Report on customer participation in the project/program.  Quantify the outputs.
  • Outcomes – Report on learning, action, and impact outcomes, quantifying outcomes and documenting results.
  • Budget – Indicate whether the budget is on-target for full expenditure within the year.
