wpe4F.gif (3193 bytes)The Community Health Assistance Resource Team (CHART) provides valuable resources and expert technical assistance that enable Missouri communities to assess and improve their community's health through local collaboration.

The CHART partnership is a coalition that represents statewide health and community-related agencies and organizations. These organizations believe in the value of health status assessment and support community development activities that are sustained at the local level. The partnership is built upon three key principles:

  • partner collaboration and participation
  • the development and availability of technical assistance for communities
  • the provision of tools and resources for communities to use to help build local coalitions and improve community health

CHART is responsible for coordinating project activities and is housed in the Missouri Department of Health. CHART provides community-specific technical assistance through personal interaction at the community level, as well as by telephone. The CHART team also provides a clearinghouse for project-related information and resource materials for communities.
