4-H vehicle license

The 4-H license plate continually promotes the 4-H name, logo and ultimately the program. Order your 4-H license plate at your local license bureau, by following three simple steps: 1. Complete the Missouri Department of Revenue (DOR) Form 1716, Application for Missouri Personalized and Special—License Plates. 2. Show your official 4-H form of identification proving that you are a member of the 4-H organization (available at your local University of Missouri Extension center). 3. Pay $15 for each license plate set ordered plus the standard vehicle license fee.
