Life Skills Survey (Word)
The survey was used by Missouri CYFAR staff to assess participant life skills.  Adapted from the Rosenberg self-esteem scale, the 4-H Four-Fold Youth Development Model, and an impact evaluation from Utah State's Youth and Families with Promise Program, the life skills questions fall into seven categories: academics and learning, communication, decision making, goal setting and achievement, problem solving, self esteem and social competencies.
