4-H PROJECT RECORD GUIDELINES

(Revised 08 - 06)

 

MUST USE THE ACAPE COUNTY 4-H PROJECT RECORD@ FORMS FOR ALL PROJECTS INCLUDING ANIMAL PROJECTS.  DO NOT USE ANIMAL FEED REPORT FORMS.

 

TOP SECTION: Be sure this section is ACOMPLETELY@ filled out.

 

GOAL SETTING SECTION: Must list at least 3 goals or more.

 

COMMUNITY SERVICE:   This section is not Project specific but list all community service projects you have participated in with your 4-H Club.

 

LEARNING ACTIVITIES:   List activities in this project this year.

 

EXPERIENCES IN LEADERSHIP: Not project specific but list all leadership activities you participated in with your 4-H club for this year.

 

FINANCIAL SUMMARY: Complete if applicable (does not apply to all Project areas)

 

APPROVAL OF THIS REPORT: All signatures must be complete

 

GENERAL RECOMENDATIONS:

- 4-HER MUST HAVE ALL SECTIONS (except for financial section which may not be applicable) completed.

 

- Report Form must be signed by Project Leader and if the parent is the project leader; they may sign the form.  EXAMPLE: This means a parent would sign on the parent line and the parent would also sign on the project leader line.

 

- Only exception is if it was a County-Wide Project meeting - then the Project Leader signature is not required but the parent signature is still required.

 

4-H STORY

- 4-H Story must be submitted.  The story must be written by the 4-H Member in the 4-Hers own words. Do NOT write one story, change just a few words and submit it for several projects.  Do NOT take another 4-Her’s story, change a few words and submit it for your project.  The story should include what you learned in this project, this year.  To receive a medal you are required to submit a short story, no more than one page, about the project with your Project Record form.

You may also submit one page of pictures to help tell the story of your project.  Pictures are optional, not required.

 

- 4-Her may hand write or type the Project Story Section.  They should be in the 4-Her’s own words.  Punctuation, spelling, and neatness will not be “graded” as long as it is readable.  Judges will judge on CONTENT not NEATNESS of Project Story.

 

          - Please NO cover pages, title pages or graphics. Please do NOT put your report forms and stories in report covers or portfolios.  Simply staple the story to the report form.
