    Description:

    The Chronic Disease Self-Management training program was developed for Leaders and Trainers for self-management of chronic conditions from over 30 years of research. The program of six (6) sessions includes methodologies, strategies and structures for successful self-management that has been validated and shown efficacy. The CDSM program to helps participants maintain living an active and fulfilling life when faced with chronic medical conditions. Key issues addressed include:
    1) Physical Activity
    2) Medications
    3) Decision-Making
    4) Action-Planning
    5) Understanding Emotions
    6) Problem-Solving
    7) Using Your Mind
    8) Sleep
    9) Communication
    10) Healthy Eating
    11) Weight Management
    11) Working with Health Professionals

    (6 Sessions) Topics: 1)Role of nutrition & physical activity in self-management. 2) Managing pain, fatigue, difficult emotions & depression. Medications & making treatment decisions.
