Chronic Disease Self-Management brings people who have similar concerns and problems. Participants are armed with tools to better manage the challenges that dealing with a chronic condition presents. Topics covered include pain and fatigue management, effective communication, goal setting, physical activity, healthy eating, stress management. Participants learn that they have a "tool box" for managing their challenges; they just have to remember to utilize the tool box.
