Chronic Disease Self-Management (Session 6 of 6)

Description:

The Chronic Disease Self-Management Program (CDSMP) is a community-based program which assists participants in maintaining quality of life when faced with chronic medical conditions. The CDSMP is provided in two and a half hour sessions, once a week for six weeks, in community settings (e.g. senior centers, churches, libraries). People with different chronic health problems attend together. Workshops are facilitated by two trained leaders, one or both of whom deal with chronic diseases themselves. Subjects covered include: 1) techniques to deal with problems such as frustration, fatigue, pain and isolation, 2) appropriate exercise for maintaining and improving strength, flexibility, and endurance, 3) appropriate use of medications, 4) communicating effectively with family, friends, and health professionals, 5) nutrition, and 6) how to evaluate new treatments. Classes are highly participative, where mutual support and success build the participants' confidence in their ability to manage their health and maintain active and fulfilling lives.

REALL Simulation

Description:

The Reality Enrichment and Life Lessons (REALL) project is a simulation designed to challenge youth to think critically about how choices and decisions made in adolescence may have consequences in adulthood. Participating youth are given the life of someone who has made reactive or negative choices (all have dropped out of high school, some have past legal histories, some have low paying jobs, etc). Following this, they are given the life of someone who has made proactive or positive choices (all have graduated from high school and obtained some type of higher education, all are employed at a living wage, all have affordable child care, etc.).

Youth are challenged to live those two lives and compare and contrast the experiences to draw conclusions in their own lives. It is the expectation that youth will draw on their experiences at the Reality Enrichment and Life Lessons simulation to make proactive choices in their own lives, so they may know self-sufficiency in adulthood.

Interstate Exchange

Franklin County 4-H enters into a 2 year agreement with a county in another state for an exchange of teens and volunteers. One year Franklin County teens and adult advisors travel to spend a week in homes of members from the partner county, and in the other year the Franklin County members host teens and adult advisors for a week in Franklin County.

Teens assist in choosing the partner county, planning and conducting fundraisers to supplement travel and hosting expenses, design exchange t-shirts, plan hosting activities, prepare a hosting notebook, and work with adults to plan and coordinate all events.
