    SNAP RECIPIENTS: Stay Strong, Stay Healthy - Level 1 (Session 16 of 16)

    Description:

    Stay Strong, Stay Healthy (SSSH) aims to provide inactive middle-aged and older adults with access to a safe, structured and effective exercise program capable of building muscle and increasing bone density, thus decreasing frailty, osteoporosis and the risk of falls. SSSH helps participants meet the Centers for Disease Control and Prevention’s physical activity recommendations.

    SSSH challenges participants through incremental increases in exercise volume and intensity. Led by a certified instructor, participants complete a warmup, a prescribed set of upper- and lower-body strengthening exercises and a cool-down at each class. Participants are made to feel comfortable regardless of their current fitness level so they can safely participate and gradually build the strength beneficial to health.
    This course is free to recipients of SNAP.

    Better Choices, Better Health - Diabetes Self-Management (Session 1 of 6)

    Description:

    The Diabetes Self-Management program is a 6-week community-based class that assists participants in managing their type 2 diabetes. This program, developed at the Stanford Patient Education Research Center, covers topics including: techniques to deal with the symptoms of diabetes, exercises for improving strength and endurance, healthy eating, meal planning, appropriate use of medication, and working with health care providers. Participants make weekly action plans, share experiences and help each other solve problems they encounter in creating and carrying out their self-management program. A self-management book, CD and refreshments included. The program is free but pre-registration is required and space is limited.

    Diabetes Self-Management Program - Clarity Healthcare May 2019 (Session 1 of 6)

    This course is designed to help people who have diabetes or individuals who care for someone who has diabetes to become better self-managers of their condition. This program will consist of six sessions each lasting 2 to 2 1/2 hours. Participants will learn to plan healthful meals with balanced carbohydrates, learn more about the benefits of physical activity, discuss blood sugar highs and lows, and how to communicate more effectively with health professionals. Millions of Americans have limited access to a supermarket, which means they must rely on fast-food restaurants, gas stations and corner stores to feed themselves and their families. People who have better access to supermarkets are more likely to eat more fruits and vegetables and less likely to be overweight or obese. Through toolkits, resources and webinars, Stock Healthy, Shop Healthy will guide any community through a unique approach to increasing access to healthy foods that involves engaging small food retailers and community members, and thus addresses supply and demand at the same time.
