Description:

Chronic Disease Self-Management Program (CDSMP) CDSMP is an effective self-management education program for people with chronic health problems. The program specifically addresses arthritis, diabetes, lung and heart disease, but teaches skills useful for managing a variety of chronic diseases. Lunch is provided at no charge

Chronic Disease Self-Management- Parkville (Session 1 of 6)

Description:

The Chronic Disease Self-Management Program (CDSMP) is a community-based program, which assists participants in maintaining quality of life when faced with chronic medical conditions. The CDSMP is provided in 2-hour sessions once a week for six weeks in community. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Be Active Your Way

Walking and other aerobic exercise is good, but it’s only one component of physical activity that should be part of everyday living. Strength and balance exercises are also important to health. In this session, we explore the types of physical activity and the impact they can have on health and overall quality of life.
