4-H Human Subjects Guide
Required steps when using the 4-H Life Skills Evaluation System

  1. Children 3rd-5th grade (junior)

  2. Youth 6th grade-17 years old, in-person (senior)

  3. Youth 6th grade-17 years old, mailed survey (senior)

  4. 18 year olds-adult participants, in-person (senior)

  5. 18 year old-adult participants, mailed survey (senior)

A. Eligibility
18 year old - adults who have participated in at least six (6) hours of program, camp or event. The life skills survey is not intended or applicable for program observers.

B. Data collection format for senior age group
In person or mailed survey.
Note: our current CIRB process does not allow a mail in survey process for the junior (3rd - 5th grade) group.

C. Data collection timing
Conduct the senior one (1) time: at the end of your program (in-person or mailed survey).

D.  Parent consent
You do not need to obtain parent consent from individuals who are at least 18 years of age. They can legally provide their own consent, even if they are enrolled in a "youth" program. See E, Participant consent.

F. Survey instrument

The senior version consists of eight (8) life skill outc
ome areas you can choose from:

1. Accepting differences

2. Communication

3. Decision making

4. Healthy lifestyle choices 

5. Leadership (senior only)

6. Self-responsibility

7. Useful and marketable skills (senior only)

8. Wise use of resources

Tip: Add the following statement at the bottom of the life skills survey to remind the participant where and when to send the completed life skills survey:  

Please mail this survey in the stamped envelope within __ days! If you lose the envelope mail the survey in an envelope WITHOUT YOUR NAME OR ADDRESS to (specialist's name, address, phone/email).

G. Optional "macro" questions that you may add to the Senior Life Skills Survey

1. Know a lot about the topics taught in this 4-H program

2. Rate this 4-H program as excellent

3. Refer my friends to this 4-H program

Required: If you want to add any other questions you will be creating a different survey than the one that was approved. As such, you will need to submit an online CIRB application to the MU CIRB that includes your version of the new survey instrument.
