Welcome!
This 4-H human subjects guide, also available as a printable Word document, was developed to help University of MO Extension faculty use a new life skill survey process. The downloadable documents in this guide are required companion pieces to the survey you will create on 4-H life skills evaluation system web site.

Background
The University of Missouri Extension 4-H Center for Youth Development purchased the the Washington State University Extension (WSUE) life skills evaluation system. In March 2009, WSUE provided MU with the newest life skills system. It was installed on a MU Extension server in April 2009. Since that time the system has been adapted, piloted and reviewed by  MU Extension campus and regional faculty and volunteers. The Missouri's 4-H Life Skills Evaluation System will be introduced to 4-H Youth Development Specialists on October 6, 2009.

Purpose of the 4-H Life Skills Evaluation System
The 4-H life skills evaluation system helps faculty measure participants' short-term life skill gains after being involved in six (6) or more hours of educational programming (i.e., a 4-H event or activities).  The system is based on high priority life skill outcomes derived from Iowa State University's Targeting Life Skills model (Hendricks, 1999). 

How does the system work?
In summary, you log on to the 4-H life skills evaluation system and select the short term life skills you would like to measure. The system creates printable life skill surveys for:

  • Juniors - children 3rd - 5th grade program participants
  • Seniors - 6th grade youth - adult program participants

You use the fillable, pdf documents in this human subjects guide to inform and recruit individuals into your voluntary survey process. You administer the survey in person or via mail in survey (seniors only). Collect the surveys and type participants' feedback in the life skills evaluation system. The system quickly generates a basic report that summarizes averages, frequencies, demographics etc. Print the report and share it with your stakeholders.

What do I need to do?
Follow the required steps listed in this human subjects guide before you create your survey in the 4-H life skills evaluation system.

1. MU Campus Institutional Review Board (CIRB) Training
The MU Extension 4-H Center for Youth Development requires 4-H faculty to protect survey participants (also known as "human subjects") through an informed consent process. This protection is carried out in the form of parent consent forms, youth assent forms and a survey process that respects the rights of everyone involved in the survey process. The CIRB training listed below provides faculty with a greater understanding of the history, purpose and importance of human subjects protection.
 

A.

Required: Make sure you have an active MU Campus Institutional Review Board (MU CIRB) account. and an active Training Compliance Certificate.

If you already have a CIRB account and training certificate go to Step 2, Terms and Acronyms.

If necessary, create an MU CIRB account at the Electronic Institutional Review Board (EIRB) web site. Resource:

After you receive email notification that you have a CIRB account you are now able to do the online training.  

B.

Required: Make sure you have an active Training Compliance Certificate.

 

 
Complete the CIRB training by signing in to the EIRB system again. Select Compliance Training (4th link down on main menu). Resource:

NEW - Select Educational Website (on the first row “Educational Training Quiz” and under the reference materials column) and resize/minimize that window to half your screen.  
Open a new Internet Explorer page and go to the MU CITI web page. Resource:

Follow the direction on the MU CITI web site:

CITI Registration Instructions

Go to www.citiprogram.org. If you are a new user, you must register in CITI. Click on “New Users Register Here” link – you will then be prompted to

  1. Choose your institution- Under “Participating institutions,” scroll through the drop-down box for
    “University of Missouri”
    ;
  2. Select your Username and Password;
  3. Enter your name: and
  4. Enter your email address.

You will then be taken to a page asking you to submit contact information; you must enter the required information (marked with an asterisk).

Once you proceed, you are directed to a page entitled “Main Menu” which shows the courses you are enrolled in for the University of Missouri. Click on the “Status” column to Enter or Re-enter the course. Upon entering the course, the “Grade Book” will be displayed which will track progress of module completion. Before beginning the course modules you will be prompted to complete the required Integrity Assurance Statement. Click and complete each module to complete the course.

You may affiliate with additional institutions, such as the VA Medical Center, during your initial registration process or at any time from the Main Menu.

Choose this Campus learner group:

  • Campus SBR - social/behavioral research

Choose this HS learner group:

  • HS SBR (social behavioral research) - those researchers doing ONLY social behavioral research

Initial HSP Training Instructions

Once you submit your contact information, you will be directed to a page entitled “Select Curriculum – University of Missouri”.

Question 1 provides options for Initial HSP Training.

Select initial HSP training or skip to question 2 if you have completed the initial or have current training with the MU IRB and are seeking continuing “refresher” training.

At any time during the taking of a group of modules, you are able to quit and return to the course later. However, you must quit after taking the quiz for a particular module, not DURING the taking of that quiz, or your answers will not be recorded. You can view your grades by clicking on “Grade Book” at the top. If you scored at least 75%, you should be able to click on the link “Return to the Main Menu to print a CITI Course Completion Certificate” or it may take you to the page automatically. There will be a link available to “View Completion Report.” This opens a new window and shows your scores, date completed, etc. for a given group of modules. PRINT THIS REPORT FOR YOUR RECORDS – THIS IS YOUR CERTIFICATION OF COMPLETION OF CITI. YOU WILL NOT RECEIVE ANY OTHER DOCUMENTATION FROM THE IRB OR CITI.

The CITI system will automatically email a copy to the IRB. We will then update the e-irb database. Once you complete the required group(s), you are certified in Human Subjects Protections for two years. Every two years you will be asked to complete a “Continuing education” component to maintain certification.
