Personal information collected from youth via the contest submission form includes: name, age, email address, mailing address, home telephone number, name of the 4-H program (if applicable), 4-H program address, 4-H program phone number, goals for submission, text/transcript of submission, signed model release form and agreement to allow University of Missouri Extension and/or University of Illinois Extension to use submission for promotional and publicity purposes.

No additional information will be collected via computer cookies or similar means.

Information collected will only be used to contact youth and the youth’s parent/guardian for more information about his or her submission, and whether or not the youth’s submission has been selected for the 4-H Digital Mayhem Challenge program. Youth and parents have the option to have material published with or without the child’s identifying information without penalty.

No information will be shared with third parties. Information may be shared with Missouri and Illinois Extension contacts for additional publicity opportunities, and information about the entry and contest outcome may be shared with your local Extension 4-H staff for acknowledgement and possible recognition. (If consent has been granted by the parent/guardian). These Extension contacts will maintain the confidentiality and security of the information. Parents have the option to agree to the collection and use of the child's information without consenting to the disclosure of the information to third parties.

The parent can review their child's personal information, ask to have it deleted and refuse to allow any further collection or use of the child's information at any time by contacting Don Nicholson and/or LeAnn Million (listed above) by phone, email or in writing.
